1.益生菌:
益生菌是一种活的微生物,当摄入足够的量时,对健康有积极影响。除了支持健康肠道微生物组的功能,如抑制有害微生物的肠道定植、帮助消化、增强肠道屏障功能和促进正常肠道运动外,益生菌似乎还有助于调节免疫功能和抑制炎症1,2。
憩室病越来越被认为是一种炎症性疾病,可能与肠道微生物生态系统紊乱有关,称为“生态失调(Dysbiosis)”3。通过帮助恢复肠道菌群平衡和减少结肠炎症,益生菌可能对急性和慢性憩室问题都有益2。

2.膳食纤维:
高纤维饮食和各种形式的纤维补充剂的几项试验表明,在减轻憩室病患者的疼痛和总体改善方面有好处14,15。
一些憩室病研究人员建议每天摄入20-30g纤维。以洋车前籽壳和磨碎的亚麻籽等物质的形式补充纤维有助于增加每日总纤维摄入量。憩室病患者如果开始补充纤维或增加膳食纤维,应在几周内逐渐补充纤维,同时增加液体摄入。这是因为过快增加纤维摄入会导致消化不良症状,如胀气和腹胀。此外,对于那些从急性憩室炎中恢复的人,可以建议低纤维饮食。在这种情况下,患者在增加饮食中的纤维,包括补充膳食纤维之前,应该咨询医疗保健提供者16-19。
3.丁酸:
丁酸是一种短链脂肪酸,由人体内消化和代谢纤维的肠道细菌产生。丁酸盐对结肠粘膜健康至关重要,是排列在结肠内壁细胞的优选燃料;有助于维持肠道屏障;具有抗炎和抗癌作用;并有助于调节结肠运动18,20,21。
在一项针对52名憩室病患者的丁酸钠随机临床试验中,受试者接受了每日两次150mg的口服微胶囊丁酸钠或安慰剂。一年后,丁酸盐组只有7%的患者出现憩室炎症状,而安慰剂组为31%22。
一项临床前试验研究了在炎症性肠病(IBD)诱导模型中直接将丁酸钙引入啮齿动物结肠的效果。丁酸钙完全阻止了诱导性结肠炎大鼠的典型体重减轻,并显著减少了结肠炎的组织损伤和液体积聚,分别减少了48%和22.7%。在本研究的实验室组成部分中,丁酸钙也表现出抗肿瘤作用23。
临床研究表明,丁酸钠形式的丁酸盐降低了肠易激综合征症状的频率,并缓解了旅行者腹泻24,25。有人提出,补充丁酸盐可能对炎症性肠病、结肠癌和某些代谢性疾病有益21。
4.抗炎补充剂:
天然抗炎化合物可能会抵消导致憩室病的一些炎症过程。其中包括鱼油(EPA和DHA),来自姜黄植物的姜黄素,以及乳香提取物等。临床试验评估这些天然抗炎剂在憩室病中的具体作用仍然需要更多开展。
- ω-3脂肪酸:EPA和DHA的一般抗炎活性已被广泛研究,许多研究表明ω-3脂肪酸对IBD有益26。在IBD患者中发现了低水平的ω-3脂肪,几乎所有使用鱼油补充剂的试验都证明了其有益效果,在溃疡性结肠炎中效果最好26,27。

在肠道炎症的临床前模型中,乳香提取物和纯AKBA都能防止氧化损伤,并防止暴露于炎症刺激的肠道内壁细胞的炎症、结构和功能变化41。一种名为AprèsFlex或Aflapin的改良提取物将AKBA与其他非挥发性乳香油脂结合在一起,与标准化为相同百分比AKBA的其他制剂相比,在较低浓度下,其抗炎活性得到了改善42。
5.谷氨酰胺:
L-谷氨酰胺是肠粘膜细胞的重要能量来源43,44。L-谷氨酰胺因其在损伤或应激(如感染或手术中发生)期间和之后保持肠道结构和功能健康以及促进肠道恢复的能力而被广泛研究。动物和实验室研究表明,L-谷氨酰胺通过保护细胞免受炎症损伤,有可能成为IBD的有用治疗药物45。由于慢性粘膜炎症和憩室病之间的关系,以及急性憩室炎发生的直接组织损伤,L-谷氨酰胺也可能有助于治疗这些疾病;然而,临床研究需要阐明L-谷氨酰胺在憩室病中的治疗作用。
6.黏胶状草本植物:
